• Diploma in Pharmacy from recognized institute.
• Age Limit : 37 Years
• Registered as Pharmacist with the Punjab
  Pharmacy Council.
• Punjabi at Matriculation Level

• 12th with science or equivalent.
• Age Limit : 37 Years
• Diploma in Multi Purpose Health Worker (Female) / ANM 
• Registered with the Punjab Nursing Registration Council 
• Punjabi at Matriculation Level

• Rs 50 Per Patient with assured guarantee of 50 Patients per day

• Rs 12 Per Patient with assured guarantee of 50 Patients per day

• Rs 11 Per Patient with assured guarantee of 50 Patients per day

• Walk in Interview

3. Click on Career and then click on “EMPANELMENT Advertisement”
4. Click on New Registration  and fill out all the mandatory details in form.
5. Upload Passport Size Photo,Signature, and other requested document.
6. Submit the dully filled form carefully as no changes will be allowed after submission.
7. After Submitting the form, take the print out for future references.
